Location: Downtown Los Angeles, CA area
Schedule available: Full-time, Monday through Friday (flexible day shift)
Pay Rate: $23 - $28 per hour (dependent on a combination of education and experience)
As a Case Manager for our permanent supportive housing program, you will play a vital role in providing support and guidance to homeless individuals and veterans. You will work directly with program participants, helping them navigate their unique challenges and develop personalized plans to achieve stable housing. Your compassion, empathy, and dedication will be instrumental in making a difference in the lives of the community.
Responsibilities:
Conduct comprehensive assessments to identify the needs and goals of program participants
Develop individualized case plans and goals in collaboration with participants
Provide ongoing case management, including regular check-ins, referrals, and follow-ups
Connect participants with appropriate community resources, such as housing assistance, employment services, mental health support, and substance abuse treatment
Advocate for program participants and assist them in accessing necessary services
Maintain accurate and up-to-date case notes and documentation
Collaborate with a multidisciplinary team to ensure holistic care for participants
Participate in team meetings, trainings, and professional development activities
Qualifications:
Prior experience in for working with the homeless and Veteran community and a genuine desire to make a positive impact
Bachelor's degree in social work, psychology, or a related field (preferred)
Relevant experience in case management or a similar role is highly desirable
Knowledge of local housing resources and community support services
Strong communication and interpersonal skills to build trust and rapport with program participants
Ability to work independently and as part of a team in a fast-paced environment
Excellent organizational and time management skills
Proficient in using computer systems and software for documentation purposes
Bilingual fluency (English and Spanish) is a plus
